Why XERF results develop over time

XERF delivers dual-frequency radiofrequency energy to three depths simultaneously: the superficial dermis, mid-dermis, and the deep SMAS fascia, the structural layer addressed in a surgical facelift. The integrated cryogen cooling protects the skin surface while the energy works below it.

The improvements that follow are not caused by the energy itself but by the biological response it triggers. Heat delivered to collagen-rich tissue initiates immediate collagen contraction and starts a repair cascade. Over the subsequent weeks and months, the body produces new collagen and elastin as part of this process. It is this neocollagenesis, the formation of new collagen, that produces the visible improvements to firmness, definition, and skin quality.

Because the process is biological rather than mechanical, results build progressively rather than appearing all at once. This is not a limitation of XERF. It is the reason results look natural and continue to improve long after the treatment itself.

  • Immediately after treatment. The skin feels slightly firmer and more taut as you leave the clinic. Some patients notice early contour refinement in the jawline or lower face within hours. This is caused by the immediate thermal contraction of existing collagen fibres. It is real, but it represents only the beginning of what will develop over the following months.
  • Weeks 1 to 4. New collagen production begins and skin starts to feel more supported from within. Jawline definition improves, the lower face feels less heavy, and skin texture begins to refine. Changes are gradual and week on week rather than sudden.
  • Weeks 4 to 12. This is the period of most significant visible change. What can XERF realistically achieve?

Setting accurate expectations is part of getting the best result. XERF is a precision skin tightening and collagen-stimulating treatment: it is not a surgical facelift, not a fat reduction tool, and not a volume restoration treatment. Understanding where it excels and where other approaches may be more appropriate helps you make the right decision.

What XERF addresses well:

  • Skin laxity and loss of firmness in the face, jaw, and neck
  • Jawline definition that has softened due to skin and tissue descent
  • Early to moderate jowling where tissue has begun to lose its structural support
  • Neck skin quality including mild crepiness and loss of tone
  • Overall skin texture and firmness including the appearance of fine lines
  • Lower face heaviness driven by structural laxity rather than fat accumulation

What XERF is not designed for:

  • Fat reduction. XERF tightens and lifts the skin; it does not remove fat. Patients where fat reduction is the primary goal are better served by fat dissolving injections or laser liposuction.
  • Advanced or significant skin laxity. Patients with very pronounced sagging may find that surgical intervention produces a more satisfactory outcome. Dr Hasaneen will assess this honestly at consultation.
  • Volume restoration. If hollowing or deflation is the primary concern, dermal filler addresses this more directly than RF energy.

How long do results last?

Results from XERF typically last 12 to 24 months. The skin continues to age naturally after treatment, and XERF does not stop this process. What it does is meaningfully reset the clock, so that even as ageing continues, the treated skin looks notably better than it would have without treatment.

Younger patients with good baseline skin quality and minimal laxity tend to see results that persist toward the longer end of this range. Patients with more established laxity or older skin may notice gradual softening from around 12 months.

What affects how good and how lasting your results are

Skin quality and collagen density at the time of treatment. XERF stimulates the body’s own collagen-producing capacity. Patients with denser, more elastic baseline skin respond more robustly and both see more significant initial improvement and sustain it longer. This does not mean older patients or those with thinner skin do not benefit, but expectations and outcomes need to be calibrated accordingly at consultation.

Degree of laxity being treated. Mild to moderate laxity responds most clearly and predictably. Patients with early-stage concerns tend to see the most transformative relative improvement because the skin still has the structural capacity to respond well to collagen stimulation. More advanced laxity may see meaningful improvement but is less likely to achieve the same degree of correction from RF alone.

Practitioner expertise and treatment precision. XERF delivers results proportional to the expertise of the practitioner. Correct depth calibration, appropriate energy settings, and precise treatment mapping across the face and neck determine how well the RF energy reaches and stimulates the target tissue layers. A bespoke, anatomy-led approach produces meaningfully better results than a standardised protocol.

Sun exposure. UV damage is one of the most significant drivers of collagen breakdown. It directly accelerates the loss of improvements gained through XERF. Daily SPF is one of the most practical steps to protect and extend your results.

Smoking. Smoking reduces collagen synthesis and impairs the microcirculation that supports skin repair. Both effects work directly against the biological mechanism that produces and sustains XERF results.
