Am I a Good Candidate for XERF?

XERF is one of the most advanced non-surgical skin tightening treatments available, but it is not the right answer for every concern.

The patients who see the most satisfying results share a few clear characteristics. Understanding them honestly before you book saves time, manages expectations, and ensures you are investing in the right treatment for your specific situation.

Here is a clear breakdown of who tends to respond best, which concerns XERF addresses most effectively, and when a different approach would serve you better.

XERF Before and After - Jawline - Dr Hass Clinic

Signs you are a good candidate for XERF

  1. You have mild to moderate skin laxity. XERF is designed specifically for patients in the early to moderate stages of skin loosening. If your jawline has softened, your neck skin feels less firm than it once did, or your lower face has begun to feel heavier without significant excess skin, XERF is well suited to your concern. The treatment stimulates collagen and reaches the SMAS layer, the structural foundation of the face, which is where early to moderate laxity is driven from.
  2. You are noticing early jowling. Jowls form as the deep fat pads of the face descend and the skin loses its structural support. In their early stages, before significant skin excess develops, energy-based tightening can meaningfully slow and improve this process. XERF reaches the SMAS fascia, the same layer addressed in a surgical facelift, making it one of the most effective non-surgical options for this concern.
  3. You are concerned about neck laxity or crepiness. The neck is one of the areas where XERF performs particularly well. Patients with mild loosening, early crepiness, or a softening of the cervicomental angle (the contour between the neck and jaw) tend to see clear improvement. If you have been told you have a turkey neck in its early stages, XERF is worth considering before the concern becomes more advanced.
  4. You want to address skin quality as well as structure. Because XERF delivers energy across multiple tissue depths simultaneously, it improves not only the structural laxity of the skin but also the quality of the dermis itself. Patients who have noticed a general loss of firmness, increased fine lines, or a less radiant and resilient skin texture benefit from this dual effect.
  5. You want no downtime. XERF requires no anaesthesia, no numbing cream, no incisions, and no recovery period. Most patients return to work and normal daily life immediately. If avoiding downtime is a priority, XERF is one of the few treatments that achieves meaningful structural tightening without any of it.
  6. You have found Ultherapy or Thermage too uncomfortable. Both established alternatives involve significant procedural discomfort requiring numbing cream. XERF’s integrated cryogen cooling system allows the RF energy to be delivered without pain relief for most patients, making it accessible to those who have avoided skin tightening treatments because of discomfort concerns. For a detailed comparison, read our article on XERF vs Ultherapy vs Thermage.
  7. You are in good general health with no relevant contraindications. XERF is a non-invasive treatment, but good general health is the baseline. Active skin conditions, inflammatory conditions, implanted electronic devices such as a pacemaker, pregnancy, and certain medications are all relevant factors assessed at consultation.

Signs XERF may not be the right choice

  1. Your skin laxity is advanced. XERF produces meaningful results for mild to moderate laxity. Where significant excess skin is present, where the tissue has descended substantially, or where jowling is pronounced and well established, the structural change required may exceed what any non-invasive device can achieve. In these cases, a surgical approach or a more invasive minimally invasive treatment such as Endolift may produce a more satisfying outcome. Dr Hasaneen will assess this honestly at consultation.
  2. Your primary concern is fat, not laxity. XERF tightens and lifts the skin; it does not remove fat. Patients whose main concern is a submental fullness (double chin), lower face heaviness driven by fat accumulation rather than tissue descent, or localised fat deposits elsewhere on the body are better served by fat dissolving injections or laser liposuction. Some patients benefit from combining XERF with fat reduction, and this can be discussed at consultation.
  3. Your concern is primarily volume loss. If the face looks hollow, deflated, or gaunt rather than loose, the issue is volume loss rather than laxity. XERF does not restore volume. Dermal filler addresses this more directly and is often the right first step before considering a tightening treatment.
  4. You are pregnant or breastfeeding. XERF is not recommended during pregnancy or while breastfeeding. Treatment should be deferred until neither applies.
  5. You have implanted electronic devices. Patients with pacemakers, defibrillators, or other implanted electronic devices are not suitable candidates for radiofrequency treatment. This is assessed at consultation.

Areas XERF can treat

XERF is not limited to the face. It can be used across a range of areas where skin laxity and collagen decline are present.

  • Cheeks and mid-face: improved firmness and skin quality
  • Jawline and jowls: one of XERF’s strongest applications; structural tightening at SMAS level
  • Neck: crepiness, laxity, and cervicomental angle definition
  • Under chin (submental area): skin tightening in this area, though fat reduction may also be needed depending on the concern
  • Forehead and upper face: skin quality improvement and mild tightening
  • Décolletage: improved texture and firmness in the chest area
  • Body areas: selected body areas with skin laxity can be treated, assessed individually at consultation

Below is an example of a patient whose XERF laser treatment was focused on the jawline, jowls, neck, submental area and décolletage.

What the consultation will establish

Candidacy for XERF cannot be fully determined without a physical assessment. At Dr Hass Clinic, the consultation with Dr Hasaneen Al-Janabi will establish:

  • Whether laxity or volume loss is the primary driver of your concern, which determines whether XERF, filler, or a combination is most appropriate
  • The degree of skin laxity and SMAS descent, which determines how significant and how lasting a result XERF is likely to achieve
  • Whether fat reduction is also needed and whether combining XERF with another treatment would produce a better outcome
  • The specific areas to treat and how they should be sequenced or combined for the most natural result
  • Realistic expectations for your specific anatomy, including what improvement is achievable and how long it is likely to last
