Most patients require between 1ml and 2ml in total, but the right volume depends on the depth of your folds, whether mid-face volume loss is contributing, and what result you are looking to achieve.

This guide sets out what to expect at each volume, how fold severity guides the dose, and why treating the nasolabial fold in isolation is not always the most effective approach.

1ml of nasolabial filler: who is it right for?

1ml is the most common starting volume for nasolabial filler and suits the majority of patients presenting with mild to moderate smile lines. At this volume, the result is a clear softening of the fold with a natural, refreshed appearance — not an overfilled or heavy look.

1ml works well when:

  • The folds are mild to moderate in depth and visible mainly at rest rather than deeply etched
  • The mid-face has good structural support with minimal volume loss in the cheeks
  • You are a first-time patient wanting to assess how your face responds before committing to more product
  • The goal is subtle refinement rather than significant volume restoration

For many patients, 1ml delivers an outcome they are completely satisfied with, particularly those treating earlier-stage lines before significant volume loss has occurred.

2ml of nasolabial filler: who is it right for?

2ml is typically appropriate for patients with deeper, more established folds, or where the skin has lost elasticity and requires more structural support to achieve adequate correction. At 2ml, the result is more pronounced and the correction more complete — but it must be placed with precision to avoid a heavy or overdone appearance around the smile lines.

2ml tends to be the right choice when:

  • The folds are deep and visible at rest, not just on expression
  • 1ml has been tried previously and delivered insufficient correction
  • There is meaningful volume loss in the surrounding mid-face area that the nasolabial fold treatment needs to account for
  • Skin laxity is present, meaning more product is needed to achieve the lift and fill that firm skin would achieve with less.

Why the fold is not always the real problem

This is the most important thing to understand about nasolabial filler — and the point most generic guides miss entirely.

Nasolabial folds deepen primarily because of volume loss in the mid-face, not because of the fold itself. As the fat pads in the cheeks reduce and descend with age, the skin loses its structural support and folds where the cheek meets the lower face. Treating only the fold — injecting directly into the crease — can soften its appearance, but it does not address the underlying cause.

For some patients, particularly those with significant mid-face volume loss, restoring volume to the cheeks first produces a more natural and more effective improvement to the smile lines than any amount of direct fold treatment. In these cases, the fold retreats naturally as the mid-face is lifted and supported.

  • Mid-face volume loss. Patients with significant cheek descent may need a combined approach rather than direct fold treatment alone.
  • Your aesthetic goal. Subtle softening requires less volume than significant correction. Starting conservatively and reviewing at two weeks is always the safer approach.
  • Filler product used. Different hyaluronic acid formulations have different densities and lift capacities. A denser product may achieve the same correction with slightly less volume than a softer formulation.

How long does nasolabial filler last?

Nasolabial filler typically lasts 9 to 18 months, depending on the volume used, the product selected, your metabolism, and lifestyle factors. Higher volumes tend to last longer because there is more product for the body to break down before the result diminishes. The nasolabial area is moderately dynamic — it moves with facial expression — which means filler here metabolises somewhat faster than in less mobile areas such as the cheeks or temples.
